Qualifications for hospice care, dictate that an individual must be certified by a physician as having a life limiting illness with a life expectancy of six (6) months or less according to the disease prograssion.
Medicare, Medicaid, and most private insurances offer benefits which reimburse that hospice and palliative care providers for the medical care provided. Hospice services are provided without regards to age, race, national origin or ability to pay.
The Management and staff of RELIANCE HEALTHCARE & HOSPICE believes that when we all work together as a team, we can accomplish more for the greater of patients and families in need of hospice care. This is achieved by maintaining a working relationship with other hospice providers by offering In-Patient and Respite Care to the patients and families of other hospice providers during a period of crisis. These services are offered as a convenience to other hospice providers and the families they serve without compromising continuity of care. Palliative Care is formally defined as care for those with life threatening and/or life limiting illness. The goal of care is to enhance
comfort and to improve the quality of an individual’s life during the end stages if a disease provess. The focus of care includes, but is not
limited to, the relief from distressing symptoms, pain management, and long term management of activities of daily living, focusing on comfort,
pain relief, and quality of life rather than the underlying disease(s).
RELIANCE PALLIATIVE CARE is a licensed Hospice facility which specializes in Skilled Nursing Care and Palliative Care Services for those with limiting and/or life threatening disease; with a special focus on pain management, wound care and symptom management due to debilitating illness: Our goal is to fill the gap for those whose needs dictate mort intensive care than those provided in Nursing Homes and Assisted Living Facilities.
PALLIATIVE CARE ensures continuity of care for those who may be transitioning from a home care or an assisted living setting in that it alleviates of eliminates stress and anxiety when additional care is needed, and the acuity of care needed does not meet the qualifications of other longer term care facilities.
Care and Services at RELIANCE PALLIATIVE CARE is under the direction of the Facility’s Medical Director who visits the patients at least once per week in conjunction with patient’s attending physican and his/her recommendations for care. Our extended care team includes:
